Safety

Medication safety is the first priority at McLaren Pharmacy Services.  Our pharmacists can coordinate discharge medications with your physician before you leave the hospital, decreasing the chance for unwanted drug duplications or interations.  McLaren Pharmacy Services can also verify that the medication you will be taking at home is the correct amount for you and scheduled at the right time.  With clinical pharmacists in multiple practice settings (hospital, retail, ambulatory care, nursing homes), we can stay abreast of our patient's complete medication history and facilitate any interventions with your physician.  Working together we can assure that you get the benefits you expect from your medications. 

Prescriptions delivered to your bedside

Filling prescriptions after a hospital stay can be a challenge.  Let McLaren Pharmacy Services take the hassle out of getting your prescriptions filled.  Ask your nurse about DischargeRx bedside delivery.  Fill out a simple order form and our in-house pharmacy will fill and delivery your prescription to you before you leave the hospital.

Convenience is key

• Start your medication(s) as soon as you arrive home.
• No need for an extra pharmacy stop, and no waiting to fill your prescription on the way home.
• We work with your physician directly to coordinate any prescription insurance issues (i.e. prior authorization).  This will prevent any delays in starting your medications.
• Your usual pharmacy may not stock the medication your physician has prescribed for you.  DischargeRx allows us to provide you with an initial supply which allows your pharmacy the time to order it.
• If you wish to continue filling your prescriptions with us, future refills can be easily filled at one of our convenient locations or called in for home delivery.
• Our inpatient and retail pharmacists work side-by-side to coordinate your discharge medication needs.  Pharmacists are available at your bedside, to answer any questions you may have about your discharge medications.

Easy to use before you are admitted to the hospital

• Please inform your pre-registration nurse or the admitting staff that you would like to use the DischargeRx program.  Please provide our staff with your prescription insurance information, or
• Complete the card below with your prescription insurance information and submit it to admitting at the hospital or mail it to us at::

Easy to use while in the hospital

• Inform your nurse, case manager or physician that you would like to use the program.
• Contact our Customer Service Coordinator anytime during your hospital stay at 517-975-6000, extension 15638 for additional information.

Delivery

• A Customer Service Coordinator will deliver your prescriptions to your bedside before you leave the hospital.
• If additional time is needed, we will arrange for your medications to be delivered to your home.

Payment

• Your usual prescription co-pays will be collected at time of delivery.
• In the event payment cannot be rendered at discharge, arrangements can be made with our Customer Service Coordinator.
